The Responsibilities of this role will include but not limited to the following:
•Coordinating Support Groups and Community Based volunteers for a strong and harmonious linkage between the community and health facility structures.
•Supervising Community Health Workers to promote retention in care and maintain continuum of care for HIV/TB prevention and treatment services.
•Supervising and undertaking community activities for contact tracing, Defaulter tracing and active case finding to halt and eliminate TB in the community.
•Undertaking and supervising community activities with Voluntary Male Medical Circumcision (VMMC) mobilisers that create awareness and increase demand as well as uptake for VMMC services.
•Conducting mentorship, onsite trainings and review performance for Community Health Volunteers and other key stakeholders in order to improve awareness and treatment outcomes.
•Monitoring the performance of all Community Program targets and review performance for Community Health Volunteers based on local agreement.
•Ensuring that all community related activities are well documented.
To be considered for this position, the candidate should meet the following minimum requirements:
• Grade 12 Certificate.
• Diploma in Public Health, Social Work, or Medical sciences.
• Certificate in Psychosocial Counselling.
• Knowledge in HIV/TB programs.
• Computer literate, with competency in using (Word, Excel, PowerPoint).
• At least 2 years’ experience in the Nursing, Clinical or Public Health Nursing profession.
• Strong Problem-solving Skills and the ability to make decision quickly.
• High level of professionalism and ethics.
